05 plate 1.9CDTI cutting out fault

Hi all have a problem that has developed the last few days, i start the van, it fires and cuts straight out, it takes about 10 attempts for it to run,and when it runs it runs lumppy and wont rev, after a few mins it starts to pick the revs up then blows white smoke out, this stops after a few revs.
things i have done.
replaced the maff for a new one seimens
removed and cleaned the egr valve.
replaced fuel filter and air filter.
once the engine is warm it will start and run no problem, on...
